Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services to children and youth. Our programs span a variety of settings, including 24/7 congregate care in one of four settings in the City of Hamilton and on-site classroom programs in partnership with the Hamilton Wentworth District School Board. We also work with and support families and partners in the community in our home and school-based programs, and our facilitated group programs.

LCC is funded by the Ministry of Health and the Ministry of Children, Community and Social Services. We are accredited by the Canadian Centre for Accreditation and licensed under the Child Youth and Family Services Act.

Thrive Child and Youth Trauma Services provides assessment and comprehensive treatment services for sexually victimized children, support to families and caregivers and programs addressing the unique mental health needs of immigrant and refugee children experiencing trauma.

We are currently looking for an **Operations and Intake Worker** to join our team in Hamilton.

**Role Overview**

Working under the direction of the Manager of Thrive Child and Youth Trauma Services, the **Operations and Intake Worker **is responsible for receiving all enquiries and referrals to programs and for providing early intervention support for clients to help ensure their immediate safety and stability. The Operations and Intake Worker also has lead responsibility and oversight of the Parent Psychoeducational Groups for clients on the waitlist for Thrive Child and Youth Trauma Services.

**Main Duties and Responsibilities**
- Receive and process program referrals, ensuring that clients meet eligibility criteria and that appropriate documentation is in place
- Initiate the engagement process with parents/caregivers and children/youth and conduct program and service orientations to new clients.
- Maintain and manage all core program client files in client information management system (EMHware)
- Manage wait list in conjunction with program Managers, ensuring that high risk and urgent referrals are triaged appropriately
- Provide crisis intervention, telephone support, education and information regarding other appropriate referrals and services (e.g. COAST);
- Participate in the review, development, and revision of referral pathways and practices between Thrive and SUSD programs and Contact Hamilton
- Arrange and attend business meetings with other agency workers and participate in community outreach, inter-agency protocol development, and committees.
- Respond to phone queries regarding Thrive programs, services and referral processes
- Other related duties in alignment with the requirements of the position.

**Requirements**:

- Completion of a related post-secondary diploma program, or equivalent in education and/or experience is required.
- Experience working with health system privacy legislation and requirements (PHIPA) and child protection legislation (CYFSA)
- Broadly based knowledge of Children’s Services, as well as a diverse knowledge of local community-based service systems.
- Flexibility to work occasional evenings and/or weekends as required.
- A valid G class drivers license and reliable vehicle is required, as this role requires local work-related travel within Hamilton. Minimum 1-million-dollar liability insurance coverage is required.
