Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including congregate care, day treatment, community-based services including in-home and school-based services, to children, youth, families and the community.

The Centre is funded and licensed by the Ministry for Children and Youth Services and accredited by the Canadian Centre for Accreditation. We are currently looking for **Full Time Mental Health Clinician, Social Worker** for our Compass Elementary School Program to join our team.

**Role Overview**

Working under the direction of the Director of Community Based Programs, the Mental Health Clinician, Social Worker is responsible to assume direct clinical responsibility for cases as assigned, including fulfilling case coordination, consultation and providing direct service to children/youth and families. The clinical therapist will also perform duties related to program development and evaluation, though developing effective linkages with service providers from larger systems in the community and act as a resource and support person to clinical teams and OT/CYW and Resource Workers as needed.

**Main Duties and Responsibilities**
- Provide direct service/counselling to the child and family and/or youth in their home, school and/or community.
- Utilization of assessments to provide recommendations and education to youth, families staff and community collaterals on treatment modalities.
- Provide resources, and treatments utilizing a wide range of treatment modalities appropriate to the needs and strengths of the child/youth and family.
- Maintain clinical records, assessments, and reports according to agency and program policy and procedures.
- May facilitate group work for children, youth and families.
- Providing clinical consultation to community schools or other programs that involve the clients.
- Liaise and support referrals with community agencies on behalf of the child/youth and family.
- Participating in the intake/screening and ongoing case planning for children/youth referred to the various programs.
- Will collaborate and engage in peer support with the Complex Needs Occupational Therapist
- Participate as a member of clinical teams which could include intake, implementation, program development and evaluation.
- Attend and participate in team meetings.
- Contribute to the development, implementation, and evaluation of the Intensive Mental Health School-based Services program.
- Contribute to effective team development and processes to promote team cohesion and communication.
- Will participate in all mandatory agency training and in-service processes.
- Assume responsibility for own professional development, including keeping abreast of new techniques in the field, community resources and acquiring new skills required to best serve the children/youth and families within the Program.
- Facilitate training sessions to staff in collaboration with the clinical director and program managers.

**Experience Required**
- Several years of progressive experience working with families and children and/or youth in a treatment/counselling setting.
- Experience working with and developing intervention plans aimed to enhance client functioning within a school setting.
- Experience working with children/youth in a school setting.
- Experience collaborating with an interdisciplinary team to set and achieve common goals.
- Experience with navigating and/or collaborating with other community systems.

**Knowledge Required**
- Knowledge of child and youth mental health and relevant evidence informed treatment interventions

**Education and training Qualification(s)**
- M.S.W./B.S.W. or equivalent with several years of related clinical experience.
- Registered and in good standing with either: CCPA, Ontario College of OCSWSSW, CRPO
- Minimum G2 driver’s License
- Certification in CHYMHs assessment or certification obtained within 3 months of hire
- SMG’s Crisis Intervention Training
