Intake Coordinator

The Alzheimer Society of Brant, Haldimand Norfolk, and Hamilton Halton (ASBHNHH) is looking for an **Intake Coordinator (13 month fixed term contract)**. Under the direction of the Team Lead, First Contact, this position serves as the initial point of contact and information for all potential clients of the ASBHNHH.
This position is located at our Hamilton Office with possibility of occasional travel. Office hours are Monday to Friday from 8:30 am to 4:30 pm, but occasional evening work may be required. The hourly rate range for this role is $20.22 - $30.33 per hour.
**Tasks**:
**INTAKE AND REFERRAL**:
1. Speaks with clients on the phone or in person (walk-in), acting as an initial point of contact and information for potential clients.
2. Screens potential clients based on standard criteria via telephone, fax and in-person contact.
3. Explores in a sensitive manner the nature of the request for service (i.e. presenting problem, request for service, precipitating factors).
4. Prioritizes cases according to needs of the client and matches clients with the appropriate program as required.
5. Schedules and reschedules appointments as required.
6. Registers clients for social groups and educational programs, and maintains, updates and monitors current client waiting lists.
7. Provides families with relevant information for other community agencies and programs if they are not appropriate for the organization, or if they require additional community services ex. legal counsel or financial planning.
8. Liaises with other community agencies to facilitate referrals and coordinate client services as required.
**DATA MANAGEMENT**:
9. Enters data into client database, recording demographic information and other client case history and performs statistical reporting as required.
**PROGRAM SUPPORT**:
10. Maintains an inventory of both internal and external resources.
11. Attends care review meetings, ASBHNHH staff and branch meetings, and other internal and external committees as required.
**OTHER**:
12. Represents the Alzheimer Society professionally in the community.
13. Participates in organizational committees and task forces as requested.
14. Complies with organizational policies and procedures.
15. Complies with the duties imposed by law or contract and the policies and procedures for performing the job in a safe and healthy manner.
16. Takes an active role in promoting and protecting personal health and safety and the health and safety of others, both staff and consumers. [Sec. 28(2) OHSA]

**Specialized Knowledge and Skill Requirements**:
- Experience performing screening and triaging is required.
- Experience working with persons living with dementia is required along with an in-depth knowledge of Alzheimer’s disease and related dementias.
- Excellent written and oral communication skills; the ability to communicate fluently in French is an asset.
- Able to work independently in a professional, self-directed, and collaborative manner.
- A valid driver’s licence and access to a vehicle are required.

**Educational Requirements**:
- A university degree in social sciences or a related field, or comparable education, along with knowledge and understanding of community resources and services is required.
- Membership in a Professional College is preferred.
