**How You Will Make an Impact**

The Clinical Intake Coordinator position at Good Shepherd Centres plays a crucial role in upholding the organization's mission, vision, and values through exemplary leadership, role modeling, and service delivery.

This role involves managing and coordinating the referral, intake, and discharge processes for the Seniors' Programs and Emmanuel House Hospice, ensuring services are delivered in a dignified, culturally-competent, and anti-oppressive manner.

You will collaborate closely with program directors, community partners, and family members to keep all stakeholders informed and up to date about the services provided. You will maintain accurate data related to all aspects of referrals, admissions, and discharges.

Responsibilities include effective communication with both internal and external stakeholders to facilitate smooth transitions and support for clients and staff. You will also play a key role in crisis response, participate in leadership meetings, and assist in the recruitment and training of clinical staff.

Additional duties include after-hours/on-call support for urgent care and operational issues, and active participation in both internal and partner agency meetings. This position is integral to ensuring that all operations align with the Good Shepherd Centres' commitment to providing high-quality care and support within their community services.

**What You'll Bring to Our Team**
- Demonstrated commitment to Hospitality as expressed through the mission and values of Responsibility, Availability, Hospitality, Adaptability, Quality and Dignity.
- Graduate of an accredited school of nursing and registered in good standing with the College of Nurses of Ontario required [Registration in College of Nurses (R.N. or R.P.N)
- 3-5 years' experience working in hospice related care, including previous palliative care experience either in hospital based palliative care or other healthcare settings with knowledge of the death/dying process.
- Current CPR certificate required or willingness to be certified
- Strong clinical assessment skills and the ability to work with evidence-based practices related to pain and symptom management and end of life care are required.
- Proven ability to work as a member of an intradisciplinary team, including evidence of supporting an environment of professional collegiality.
- Resourceful problem-solver with excellent communication and interpersonal skills required to effective provide care in an interdisciplinary setting, excellent time management and ability to perform assigned duties with attention to detail.
- Demonstrated commitment to professional continuing education.
- Proficient in computer software such as Word, Excel, PowerPoint
- Good organizational skills and ability to prioritize workload.
- Hospitable in dealings with residents/caregivers / volunteers / staff, and other key stakeholders.
- Demonstrated ability to adapt in a fast paced and changing work environment.
- Respectful, collegial and compassionate demeanor.
- Valid driver's license and access to a reliable vehicle.
- To observe all Good Shepherd health and safety rules and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations to minimize risk to residents, visitors, volunteers and staff as well as to the property of Emmanuel House
