**WHAT YOU’LL DO**:
Social work provides service to patients and their families focusing primarily on the psychological and the emotional well-being of individuals as they adjust to the changes in their health. Social Work advocates on behalf of patients and families as they move through the health care system. Responsibilities include: biopsychosocial assessment, problem formulation, assisting in the development, implementation and ongoing monitoring of patient treatment plans, crisis intervention, and knowledge of community resources. In addition, the social worker provides information, consultation and education to patients, families, staff and community agencies on matters relating to their specialty. The Social Worker practices in accordance with the OCCSWSSW Professional Standards.

**WHAT YOU’LL BRING**:

- Master’s Degree in Social Work required
- Member of the Ontario College of Certified Social Workers and Social Service Workers (OCCSWSSW) in good standing
- 3 years recent relevant clinical experience working with patients and their families living with chronic illness
- Certificate in “Critical Incident Stress Debriefing” preferred.

**WHO YOU ARE**:

- Demonstrated commitment to providing services for patients and families, which address health from a holistic perspective (including physical, emotional, social, economic, spiritual, and environmental influences on health).
- Must be able to advocate on behalf of patients, families and/or the health care system.
- Ability to work independently without supervision.
- Ability to organize workload and establish priorities effectively and adapt to unpredictable and changing situations.
- Demonstrated exceptional interpersonal, verbal, and written communication skills and judgement when relating to adult clients, families, co-workers, and members of the interdisciplinary team.
- Demonstrated expertise in crisis and trauma counseling with individuals and/or families.
- Demonstrated expertise in grief theory and therapy.
- Demonstrated expertise in assessment and counseling with non-compliant patients and/or their families.
- Demonstrated experience with psychological assessments, complex discharge planning, crisis intervention and adjustment counselling.
- Excellent counselling skills and knowledge of community support services; Demonstrated knowledge of, and commitment to the principles of chronic disease prevention and management.
- Demonstrated ability to build and maintain effective relationships with a diverse range of people and work effectively within an interprofessional team;
- Demonstrated problem solving skills and ability to exercise independent judgement and to assess situations and address problems efficiently and effectively; and
- Demonstrated understanding of and commitment to comprehensive safety programs and practices including staff, client and environmental safety and participation in regular in-service education in this area.
- Computer literacy, including knowledge of computer programs such as Microsoft Office and Internet Explorer
- Experience using Epic considered an asset
- Experience within an outpatient clinic-based setting considered an asset
- Fluency in a second language considered an asset
