**ABOUT THE ROLE**

Reporting to the Manager - Regional Rehabilitative Care Program (RRCP), the Rehabilitation Assistant (RA) working with the Community-based Stroke Outreach Program will provide therapy (in person and virtually) within a regional outreach service specific to clients with stroke and those with chronic diseases. The Community-based Stroke Outreach Program is a pilot service designed to:

- Provide access to community-based stroke rehabilitation for the region East of Thunder Bay (ie. Nipigon, Terrace Bay, Manitouwadge, etc).
- Services will be both in person in the community the client lives as well virtual. In person will be a combination of outpatient, partnering with the local hospital/First Nation Home and Community Care, and providing services in the client’s home.

The RA is responsible for carrying out elements of treatment programs under the direction/supervision of the Occupational Therapist and Physiotherapist as directed by regulatory/professional practice standards and/or guidelines of the supervising health care professional. The RA may also be supporting elements of the virtual Speech Language Pathology treatment plan, and or Stroke Navigator plans.

Tasks are assigned based on consideration of the client providing consent for treatment by the RA (as supervised by the healthcare professional), the client’s status, related environmental factors, and the supervising health care professionals’ and RA’s level of competency (skills, knowledge, and abilities) to perform these tasks. Interventions may take place in the local hospital, client’s home, or other community setting. This is a pilot program and will include participation in evaluation and recommendations for the program going forward.

**WHAT YOU BRING**

Education/Experience:

- Diploma/degree from a recognized two-year Rehabilitation Assistant program (equivalent combination of a similar post-secondary program focusing on physical rehabilitation (e.g. Kinesiology) and experience working in rehabilitation assistant capacity will be considered).
- Previous experience working directly with clients in a healthcare facility is preferred
- Working knowledge and experience with theories and principles of guiding adult learning and client self-management approaches

Skills/Abilities:

- Ability to communicate effectively and concisely, both orally and in writing;
- Ability to establish effective working relations with supervising therapists, coworkers, inter-professional team, and clients
- Excellent time management, analytical/critical thinking, problem-solving skills
- Ability to work independently and as part of a team
- Must be at a basic skill level in word processing and spreadsheet software
- Demonstrated regular attendance

Assets:

- The ability to converse in a second language would be an asset

Conditions of Employment:

- This position shall be conditional upon a satisfactory **Police Vulnerable Sector Check (PVSC)**where indicated, to ensure the absence of relevant criminal convictions.
- Will be required to travel to various work locations throughout the District of Thunder Bay (East of Thunder Bay i.e. Nipigon, Geraldton, etc.) and remote First Nations north of Sioux Lookout, and provide own reliable transportation.
