**Summary**:
The clinical support coordinator collaborates closely with clients, clinicians, and community partners to ensure seamless access to services, while supporting healthcare providers through advocacy, outreach, and operational efficiency. This position connects with those referred to the program to provide program overview, assess self-management and readiness, and identify challenges that could be addressed prior to the initial assessment of the client. Key responsibilities include coordinating healthcare initiatives, analyzing program effectiveness, and assisting with policy changes. Additionally, the clinical support coordinator engages in outreach to promote services and foster relationships with diverse groups of stakeholders.
**Duties and Responsibilities**:
- Supports clients and program clinicians in all aspects of day-to-day activities
- Coordinates program initiatives in alignment with provincial and national healthcare standards
- Liaises between clients, clinicians, and support services to improve care outcomes
- Contacts clients referred to the program to provide program overview, assess self-management and program readiness, and identify potential challenges to address before initial assessment
- Supports clients by facilitating access to appropriate care and resources
- Collaborates with healthcare professionals and community organizations to optimize program delivery
- Analyzes program effectiveness and makes recommendations to improve service efficiency and reduce community strain
- Assists in the implementation and adaptation of policy changes affecting client services
- Maintains detailed records and documentation in compliance with privacy and data protection standards
- Engages in ongoing outreach activities to promote services and build relationships within diverse communities
- Fosters effective relationships across health authorities, community organizations, and government bodies
- Seeks input from diverse voices including clients, Indigenous communities, healthcare providers, and policy makers
- Explores emerging health trends, technologies, and community needs to inform responsive and forward-thinking initiatives
- Communicates and acts in a respectful and professional manner, collaborates effectively, abides by policy, and contributes to a positive and productive work and learning environment
- Engages in continuous learning by participating in activities, training, and events related to Indigenous engagement, reconciliation, equity, diversity, and inclusion

**Qualifications**:
- A bachelor’s degree in healthcare, social work, or a related field
- Minimum of two years’ experience in project or program management, client advocacy, case management, or a similar role involving healthcare professionals in a healthcare setting
- Comprehensive knowledge of the provincial and national healthcare systems, local and national policies impacting client services, as well as community resources and services
- Strong understanding of medical terminology
- Skilled in conducting outreach and engaging with the community
- Excellent interpersonal and communication skills
- Ability to handle sensitive and confidential information with empathy and discretion
- Understanding equity, diversity, and inclusion principles
- Demonstrated ability to work effectively both independently and collaboratively with diverse populations
- Strong problem-solving, prioritization, and organizational skills are important
- Fluency in multiple languages is an asset

**Salary**: The salary range, based on 1.0 FTE, is $69,035.00 - 107,868.00 per annum. The starting salary will be commensurate with education and experience.
