- functionally with nursing professional practice, the position plays a key role in promoting and continuing to develop a strong patient safety program throughout the organization, and will keep abreast of current literature in patient safety. The position will work closely with the Leader, Patient Safety, Quality and Clinical Risk Management.

**Duties**:
**Patient Safety Program**
- Promotes and supports safe medication practices within an overall safety culture at the Royal, working with the Leader, Patient Safety, Quality and Clinical Risk
- Ensure the Royal’s medication safety practices are in alignment with Accreditation Canada requirements and relevant college standards and legislation
- Engage and support the empowerment of patients to be partners in healthcare
- Coordinate and promote organization-wide adoption and dissemination of leading evidence-informed practices in medication safety
- Contribute to continuous quality improvement by leading quality improvement initiatives related to patient safety and safe medication practices
- Develop and support the implementation of policies and procedures to support patient safety
- Working in close collaboration with Nursing Professional Practice, support and deliver education on safe medication practices, and coaching on strategies for patient safety regarding medications.
- Share pockets of excellence and learnings from incident analysis and process changes made to promote patient safety outcomes in all clinical programs
- Share trends and aggregate medication incident data and analysis of incidents with clinical leaders.
- Share data and key medication safety practices with medical and pharmacy leaders as needed.
- Proactively educate staff regarding regulatory issues, new guidelines, medication safety, and risk activities.
- Assume occasional temporary duties of the Leader, Patient Safety, Quality and Clinical Risk Management to cover patient safety activities
- Attend patient safety huddles throughout the organization.
- Support the annual prospective analysis, from idea to change implementation
- Liaise with Quality & Patient Safety peers and colleagues in the region and beyond
- Ensure a work environment that is conducive to the Royal’s Anti-Racism, Harassment and Discrimination Free policy
- Provide support as needed for the patient safety incident reporting system
- Review all reported medication incidents and participate in investigations as required
- Participate and/or lead critical incident quality of care reviews and root cause analysis as required and collate and report findings for various stakeholders
- Recommend and facilitate change within the Royal to improve patient safety, based on identified risk
- Additional duties as required by the Director, Quality & Patient Safety.
- Travel between the organizational sites on a regular basis.

**Qualifications**:

- Undergraduate degree and clinical experience within the nursing discipline. Masters level degree in Public Health, Healthcare Administration, Nursing, or similar program is an asset.
- Current registration with the College of Nurses of Ontario.
- Minimum five years relevant clinical experience
- Experience in patient safety and/or risk management preferred
- Operational knowledge of professional practice standards, statutes and regulations governing the operation of a mental health public hospital and pertaining to patient care delivery and patient safety, including medications, medication safety principles and medication administration requirements and standards.
- Knowledge of patient safety theory and tools used in a healthcare environment such as use of root cause analysis, failure mode effects analysis, control charts etc.
- Knowledge of adult education techniques
- Experience coaching and facilitating groups, and individual coaching.
- Project management experience is an asset
- Excellent written and verbal communication skills to be able to produce reports and deliver presentations to program teams, as well as senior management.
- Demonstrated change management, problem solving, and data analysis skills are an asset.
