Pathways to Housing**program supports program participants who have experienced barriers to housing with significant physical and/or mental health challenges. Participants are provided a housing subsidy and rapid access to a myriad of supports through an Assertive Community Treatment team to encourage stability, recovery, and improved quality of life. Pathways Assertive Outreach (PAO)**program is a non-housing support service that is an evidence-based Assertive Community Treatment (ACT) team that practices within the ACT Fidelity framework and is guided by the Recovery and Assertive Engagement principles**.**The PAO program partners with other housing providers to support**stability, recovery, and improved quality of life.

Reporting to the Director of Primary Care, the Clinic Manager, Housing & Clinical Services provides strategic leadership that fully integrates housing stability and clinical care. The role directs all operations for Pathways to Housing and PAO, overseeing multidisciplinary staff, budgeting, program design, and physician engagement while embedding rigorous quality-assurance and data-driven outcome evaluation. By aligning program deliverables with organizational strategy and funder requirements, the Clinic Manager, Housing & Clinical Services ensures The Alex delivers holistic, person-centered services that measurably improve client health and long-term community well-being.

This is a permanent, full-time position.

**What You Will Do**

Operational Leadership
- Direct the day-to-day and strategic operations of the Pathways to Housing program and Pathways to Assertive Outreach (PAO).
- Oversee budgets, meet all funder reporting requirements, and support fund-development activities (tours, data requests, donor information).
- Lead external communications related to clinic and mobile services.

Team Management
- Lead day-to-day operations of a multidisciplinary team (recruitment, orientation, scheduling, payroll, recognition, and performance management).
- Coordinate staff onboarding/off-boarding and support ongoing professional development for all staff.
- Foster a culture where all staff practice to full scope, receive mentorship, and pursue professional growth.
- Oversee workplace health & safety, privacy, and EMR stewardship.
- Coordinate staff and physician onboarding/off-boarding and support ongoing professional development for all staff.

Strategy & Governance
- Set program vision, goals, and measurable outcomes aligned with The Alex’s strategic plan.
- Develop annual action plans and budgets; monitor performance against KPIs, accreditation standards, and funder requirements.
- Update policies, practice guidelines, and quality-improvement initiatives to drive service excellence.

Clinical & Housing Integration
- Facilitate seamless collaboration among clinicians, housing specialists, and community partners to deliver trauma-informed, evidence-based care.
- Manage client intake and exit decisions; ensure timely, client-centered transitions along the housing and clinical continuum.
- Partner with Team Leads/Assistant Team Leads to enhance workflow efficiency and client outcomes.

Quality, Evaluation & Reporting
- Establish program metrics; analyze EMR and operational data to identify trends and drive continuous improvement.
- Prepare reports for internal stakeholders, funders, and accreditation bodies.
- Facilitate the development, documentation, and continuous improvement of program guidelines, workflows, and policies in collaboration with staff and stakeholders to support effective, values-aligned service delivery.

Advocacy & Community Engagement
- Represent The Alex in community forums, sector networks, and internal leadership meetings.
- Champion the needs of vulnerable populations and promote The Alex’s values, vision, and mission.
- Perform other duties as assigned.

**What You Bring**
- Current licensure in a regulated health profession (e.g., Registered Nurse or Licensed Practical Nurse) supported by the required post-secondary training.
