**Position Summary**:
The Community Concurrent Disorders Program (CCDP) is a wraparound service for individuals experiencing mental health and substance use issues. Trillium Health Partner’s short-term case management program utilizes a person centered approach that is grounded in harm reduction and recovery principles.

**Key Qualifications**:

- Professional qualification in a health discipline with addictions specific training required.
- Certification through the Canadian Addiction Counsellors Certification Federation considered an asset.
- Minimum 2 years experience working with adult clients with mental illness and substance use issues.
- Demonstrated ability to assess clients, implement a service plan and facilitate referral to treatment programs within a short -term model of care.
- Experience with utilizing motivational interviewing skills and techniques.
- Knowledge of community resources.
- Excellent communication, organization, presentation and problem solving skills.
- Valid driver’s license, relevant insurance and use of own vehicle.
- Demonstrated ability to work independently and cooperatively with community partners.
- Ability to speak a second language is considered an asset.
- Proficient computer skills.
- Reliable, flexible and collaborative team player.
- Positive work and attendance record essential.
