**_Intervention Case Manager_**

**General Job Description**:
The Intervention Case Manager will assist clients to meet their needs and enable them to utilize agency or community services to achieve their optimal level of health through counselling, education, and the use of appropriate resources either independently or through a combination of any of the above.

**Qualifications**:
BSW or equivalent preferred. Registered, or eligible for registration with the college related to education. This position reports directly to the Director of Programming.

Must be familiar with mental health, harm reduction, gender identity, gender-based violence, homelessness, and sexual health.

**Job Status**:
Full Time. This position is contingent upon ongoing approved funding through the North

East Local Health Integration Network and/or other sources. A valid driver’s license and access to a road worthy vehicle are requirements of this position. RIWC requires that all employees be fully vaccinated against Covid-19.

**Responsibilities**:

- To provide assessment, case management, safety planning, and treatment planning to clients either in person or via virtual technology and/or telephone.
- To fast track and streamline high risk/needs clients to mental health services without a wait list or need to wait for an intake appointment.
- To offer support to individuals who are in immediate need, unsafe or unable to wait for later appointments and thereby remove barriers to wrap around mental health support and advocacy.
- Flexibility to work outside of the office setting i.e., hospital, community, partner agencies, client homes etc.
- To consult with and utilize appropriate resources from community, social, health and welfare agencies and provide referrals as appropriate including case conferences.
- To maintain accurate client records and statistical reports as required
- To maintain the confidentiality of all client information
- To develop educational programming that addresses the needs of the community
- To attend and participate in staff and agency meetings
- To work as a team member
- Perform all other duties that may be required from time to time as requested by the Executive Director.
