This role is part of an exciting program currently being launched at SEED, the EDGE Program (funded by OSD). We’re looking for individuals who are comfortable with change, eager to contribute to a new initiative, and ready to help shape its success from the ground up.

The EDGE Program is an early intervention program that provides targeted employment focused programing for youth (ages 18-26), who are connected to Employment Support Income Assistance (ESIA), or at risk of becoming connected. As the program is delivered online, the Case Manager will engage with youth in a virtual setting.

Reporting to the Program Manager, the Case Manager is responsible for all case management of youth. They will meet one on one with youth, complete assessments, identify areas of strengths and weaknesses and work with them to determine and work towards their employment, life and social goals.

**Duties**:

- Case manage approximately 50-100 youth on an annual basis including development of personalized intervention strategies
- Review youths’ background information; complete intakes and assessments
- Assess youths’ strengths and needs; work to identify and help decrease barriers to success (i.e. poverty, education, home life, life skills, etc.)
- Work with youth and other members of the EDGE team to build a case plan based on their goals and needs.
- Meet with youth every 1-2 weeks to provide support and to work on goals; assess progress and discuss any difficulties
- Work to identify and address any obstacles that might hinder a youths’ progress (i.e.. childcare, transportation, or communication issues)
- Develop partnerships and collaborate with community to support the development of services for the target population.
- Refer youth to other social services or community providers where necessary(i.e. legal, medical, financial assistance, housing, transportation, day care etc.)
- Maintain contact with other social service agencies involved with the youth to provide information and obtain feedback on overall progress (within the boundaries of privacy protocols).
- Maintain program statistics for purposes of evaluation and research and provide details to program manager for LAMPSS reporting.
- Maintain timely and accurate records to satisfy Department of Opportunities & Social Development (OSD) requirements and internal reporting processes.
- Attend weekly case management meetings with all EDGE staff to discuss youth progress and updates
- Represent SEED within the network of youth focused service providers
- Must be willing to cross train and assume other duties as needed

**Qualifications**

Experience & Education
- Post Secondary education in social work, child and youth care, psychology or other social science or health-related discipline.
- Minimum of five (5) years of experience (or a combination of education and experience) providing case management support to marginalized and at-risk youth
- Experience working with clients attached to ESIA, including knowledge of related barriers and government policies
- Ability to recognize existing barriers to employment frequently present with OSD clients (e.g., poverty, education, home life, life skills)
- Strong ability to assess job readiness

Skills & Competencies
- Experience meeting in a virtual environment
- Demonstrate understanding of different learning styles and their impact on participants’ engagement
- Ability to adapt intervention strategies to meet individual client needs
- Strong communication, interpersonal, and presentation skills
- Excellent organization and written communication skills, with the ability to maintain accurate and timely case notes
- High level of initiative; able to work independently and manage unanticipated issues
- Demonstrated discretion and sound judgment in handling confidential information

Community Engagement
- Excellent knowledge of community supports and resources
- Ability to build relationships and connect youth with appropriate services
