**Team Lead, Patient Flow Coordinator - Intake**
**Position summary**:
The Team Lead, Patient Flow - Intake is responsible for playing a lead role in coordinating contact between the referring organization, inter-professional team, patient and/or Substitute Decision Maker (SDM). The Team Lead works collaboratively to facilitate a safe and efficient admission process, ensuring all patients are admitted and referred to the most appropriate programs/services for their clinical needs.
**Responsibilities include**:
- Conducts pre-admission interviews and clinical assessment at referring facility to evaluate appropriateness of referred patients based on established program’s inclusion and exclusion criteria
- Conducts pre-admission tour for patients/SDM accepted for admission, as required.
- Attends daily Bed Meetings with partner organizations, cascades information as appropriate to team members
- Develops/enhances tools to monitor and analyze the admission process including collection of data pertinent to Hospital’s admission criteria.
- Develops policies/procedural guides to improve and enhance patient admission.
- Responsible for data collection and reporting related to Ministry requirements.
- Plans, coordinates and manages inpatient referrals and admissions to RHC in alignment with organisations strategic direction and operational needs
- Coordinates the date and time of patient admissions or transfers with the referring/receiving facility.
- Facilitates re-evaluation of complex patient admissions and program change requests, working with the interprofessional care team
- Monitors and tracks patient admission identifying themes and trends for each program, compiles reports and provides recommendations as required
- Attends complex discharge planning meetings
- Liaises with inter-professional team members as required to plan a safe and effective transition of care.
- Meets with the patient/SDM on the day of admission to complete the required admission forms.
- Communicates patient information to the clinical team and support services as appropriate, to facilitate a safe and efficient admission.
- Minimum of an undergraduate degree in healthcare related field required
- Member in good standing with a professional healthcare regulated regulatory college required
- Minimum (4) years clinical experience preferably in a Rehabilitation/CCC setting, leadership experience preferred
- Demonstrated expertise with geriatric, neurological and MSK conditions and have Current Driver’s License
