**Role Description**:
Reporting to the Population Health Services Program Manager, the Health Promotion Coordinator will support all aspects of Population Health Services, including health protection, health promotion and health assessment, with a very strong emphasis on health promotion work.

**Roles and Responsibilities**:

- Identify and recommend strategies to build health promotion initiatives.
- Oversee, guide, and supervise the community health workers.
- Liaise online and in-person with community agencies, shelter partners, and ICHAclinicians to develop health promotion services.
- Develop interactive, engaging, and inclusive activities to support health promotion effortsacross the sector.
- Assist in immunization campaigns for vaccines, STBBI, and harm reduction services.
- Work closely with shelter personnel, ICHA clinicians and clients to implement health promotion and outreach activities in collaboration with the Population Health Service.
- Support the development, implementation, evaluation, and reporting of health promotion strategies.
- Develop and execute education initiatives to support population health interventions for ICHA’s clinical and non-clinical teams.
- Support and develop other population health projects and initiatives.
- Other tasks as assigned.

**Qualifications**:

- At least 2-3 years of experience in health promotion and team supervision or equivalent experience.
- A master's in public health with a specialist certification in Health Promotion an asset.
- Experience with developing and evaluating health promotion initiatives.
- Proven ability to work effectively with a broad interprofessional team and multiple partners in a complex organizational and community environment.
- Self-directed with proven ability to meet deadlines in a demanding environment.
- Knowledge and experience using electronic medical records (EMR).
- Experience working with individuals and families experiencing homelessness and/or those who face structural vulnerabilities, systemic oppression, and marginalization.
- Solid organizational, project management and time management skills; ability to support multiple clinical sites and processes concurrently, continuously identifying and addressing competing priorities in a timely and effective manner.
- Excellent communication skills, both written and verbal.
- Strong critical thinking and problem-solving skills.
- Excellent computer skills (including Microsoft Office) and an ability to learn new systems.
