About the Role:
The Health Promoter role focuses on enhancing the health and well-being of the 2SLGBTQ community. This role involves planning, implementing, and evaluating community-driven health promotion initiatives that address the social determinants of health, reduce health disparities, and enhance access to culturally competent services for 2SLGBTQ populations. The Health Promoter will provide support to all aspects of the health promotion activities, including outreach, program implementation, and evaluation.
Scope of the Mandate:
The Health Promoter will develop and implement targeted health promotion initiatives that are inclusive, evidence-based, and responsive to the specific needs of the 2SLGBTQ community. This role includes conducting community situational assessments, coordinating health education activities, and engaging in policy advocacy to address system barriers and promote equity in health outcomes. The Health Promoter will engage in case management tasks, working collaboratively with service users to conduct assessments, establish clear goals, and coordinate care considering the social determinants of health.
Activities will be implemented at individual, community, and policy levels to increase access to equitable health services and support community-led initiatives that foster resilience and well-being within 2SLGBTQ communities. The Health Promoter will leverage research evidence, community feedback, and best practices to guide the development of programs, resources, consultations, and policy briefs.
Day-to-Day Responsibilities:
Health Communications / Knowledge Translation:
- Develop and disseminate culturally relevant health communication materials tailored to the needs of 2SLGBTQ communities.
- Translate complex health information and research findings into accessible information that can inform community action and policy advocacy.
- Act as a resource for program inquiries, providing accurate information and connecting community members to appropriate services and supports.
Program Planning, Implementation, and Evaluation:
- Utilize an understanding of public health sciences to inform and guide health initiatives for the 2SLGBTQ community.
- Conduct community-based research and situational assessments to identify health needs, barriers, and opportunities within 2SLGBTQ communities.
- Design and plan health promotion initiatives that are evidence-informed and tailored to the unique aspects of 2SLGBTQ health and well-being.
- Evaluate program outcomes using quantitative and qualitative methods, ensuring continuous improvement and responsiveness to community needs.
- Provide capacity-building training and consultations to community members, students, and others on the needs and issues of 2SLGBTQ communities.
Outreach, Engagement, and Partnership Development:
- Identify and leverage existing community resources to support the success and impact of the health promotion program.
- Develop and implement outreach strategies that increase engagement and participation in health promotion programs across 2SLGBTQ communities.
- Support the development of partnerships with local community organizations, health departments, schools, businesses, etc.
- Collaborate with partner organizations to share relevant information and resources, ensuring consistent communication and capacity-building.
Case Management:
- Perform intake assessments, work with service users to identify goals, and develop a plan that supports the service user in achieving their goals.
- Facilitate coordination, communication, and collaboration with service users and partners to support service user goals.
- Maintain records in the electronic medical recording system, in accordance with Sherbourne policies and procedures and privacy legislation.
Administration:
- Work with the Program Supervisor, Coordinators, and Senior Manager to identify and prioritize issues, plan overall program goals and objectives, and evaluate their effectiveness.
- Collaborate with team members to plan and implement Health Centre initiatives and support established policies and procedures.
- Make recommendations and contribute to policy formulation and quality improvement initiatives.
- Participate in the development, implementation, and evaluation of programs and standards of practice for service delivery and accreditation.
To Thrive in This Role, You’ll Need:
Education/Experience/Knowledge and Skills:
- Education in public health, health promotion, or health/social sciences at a Master’s level or equivalent formal training.
- Demonstrated knowledge of 2SLGBTQ+ populations, specializing in trans and non-binary health.
- Excellent written and oral communication skills, including writing for various print and social media.
- Significant experience in group facilitation, community education, and training.
- Good administrative and organizational skills, including knowledge of computer software, project and event management, and report production.

Working Conditions:
The role requires the ability to function with minimal direction, organize complex workloads, and be flexible in responding to the organization's needs. Some evening and weekend work is required.
