**HOURLY RATE**:$32.92

**GENERAL DESCRIPTION**:This program provides continuing care supports for 20 -25 self-identified women who have recently stayed at the MGH WMS. Participants will be offered a range of services and supports based on their individual goals including: short-term case management, accompaniments, peer support, supported referrals, groups and social-recreation opportunities. This program is rooted in a women-centered, trauma informed approach, working from a harm reduction framework, to support self-identified women across the continuum of care with particular focus on critical transitions. Project staff will work in partnership with all WMS staff, as well as external providers, to ease this transition and ensure continuity of care. This position reports to the Supervisor, TCAT Case Management and is part of CUPE 7797.

**MAJOR DUTIES**
- Provide direct community connection and case management support to identified individuals, working from our integrated model of care
- Act as a liaison and advocate across providers, responsible for linkages and coordination of care to ensure comprehensive continuous care planning and follow up supports
- Working closely with the research/evaluation and leadership group to ensure this program is delivered with a gender equity and trauma informed lens, which may require capacity building components amongst partners
- Support and participate in the implementation and ongoing evaluation components of this pilot project
- Conduct intake and assessment interviews with participants.
- Provide case management, completing detailed assessment, coordination of supports, referrals and advocacy on relevant health issues, substance use, mental health, financial, employment, housing, legal and social supports.
- Facilitate ongoing coordination of care with MGH Withdrawal Management Services, liaising with all providers involved
- Work with clients to identify and achieve recovery goals as identified by individual service users.
- Maintain ongoing support in the community, including home visits, office visits, accompaniments to appointments, and meetings in institutional settings.
- Work collaboratively and develop strong partnerships with external service providers, such as withdrawal management services, treatment programs, hospitals, law enforcement agencies, and housing providers.
- Connect clients with supports in the community based on their needs which will help them achieve and maintain stability, including support groups, life skills groups and social events.
- Develop and ensure the provision of relevant groups and activities for the client group as appropriate.
- Intervene and deescalate crisis situations.
- Engage in the active team case management and clinical consultation meetings.
- Maintain accurate records and complete reporting required by funders and supervisor, adhering to PHIPA and TNG-SSCH policies.
- Take part in research activities as required, including collection of necessary service data to support the project functioning and quality improvement activities.

**REQUIREMENTS & QUALIFICATIONS**:

- BA/B.S.W plus 2 years of recent experience in human services, preferably as a case manager or a combined equivalent of education, lived experience and/or significant related training.
- Minimum 2 years’ experience working with individuals living with mental health, substance use issues, histories of trauma and homelessness
- Experience, alignment and commitment to harm reduction
- Strong familiarity and commitment to women-centred and trauma informed care
- Recent demonstrated knowledge and skills in assessment, crisis intervention and prevention, conflict resolution and negotiation skills
- Certification in Conflict Prevention and Intervention or willing to be trained

**Job Types**: Full-time, Permanent

Pay: $32.92 per hour

Expected hours: 35 per week

Schedule:

- Monday to Friday
- Weekends as needed
