Overview The Case Manager is responsible for developing and overseeing case management, and providing case management leadership within youth’s circle of care. This position is responsible for all case management functions to best meet the youths’ needs, including relationship building, assessment, collaborative planning, monitoring/evaluation, advocacy, and systems navigation support involving internal and external services and resources. This position will work in collaboration with team members across our continuum of services, and utilizes an integrated Client-Centered, Trauma Informed, Harm Reduction Informed, AOP, Housing First for Youth, Strengths/Resiliency based approach, with attention to developmentally appropriate wellness and transition plans towards sustainable exits from homelessness. Responsibilities Providing intensive case management services through a community-based, client-centered, and strengths-based lens that promotes stabilization, increasing access to housing, income supports, community building, primary care provider and mental health supports. Meeting with youth to collaboratively assess their needs and develop a case plan that meets youth’s goals, reflects youth’s strengths, and enhances resiliency and positive youth development. Building therapeutic relationships with youth, ensuring ongoing and personalized support, which contributes to the broader goal of community stabilization and empowerment. Facilitating connections to community resources, identifying service plan goals, and providing assessment and referral services, counseling, service navigation, and follow-up support, which aligns individual needs with systemic resources. As part of individualized case planning, collaborate with HART Hub’s service pathway for access to supportive housing units and housing support available through internal referral process. Collaborating with community partners including HART Hub Intake worker and team to deliver coordinated and effective services, bridging the micro-level individual needs with macro-level systemic solutions. Assess risks and develop safety plans in collaboration with youth (e.g., relational, mental health, substance use, immigration concerns) and coordinate proactive risk mitigation strategies. To work with youth through a trauma informed lens by fostering peer mentorship, increasing their capacity for self-advocacy, and helping build a strong recovery support network. Applying a social work lens when working with youth, offering emotional support and intervention strategies such as grounding techniques to promote emotional regulation, addressing individual mental health within the context of societal influences. Overseeing collaborative implementation of Case Plan support steps with identified support network and other agency staff; monitoring, evaluating and mentoring interventions as needed to actively help support youths’ progress. Providing case management support that ensures youth have access to appropriate resources, timely referrals, and coordinated pathways to internal and external service providers—addressing both case management and clinical needs, including those related to physical health, mental health, substance use, and cognitive development. Conducting regular case plan reviews, updating documentation of progress, next steps, and goal revision by directly reviewing with youth, consulting with designated Consistent Workers and others in circle of care; chairing Case Management Team review meetings. Coordinating case conferences and consultations with internal and external client supports as needed. Knowledge of community resources for mental health, substance use, community resources for housing, supportive housing and shelters. Ability to work closely with teammates and participate effectively as part of the larger program and organization. Ensuring all case documentation/client files, agency program documentation, and data collection are completed and maintained according to agency and professional standards. Participate actively in program development, interdepartmental collaboration, supervision, and continuous learning. Contribute to quality improvement initiatives and evidence-based practice within the department. Adhering to the CASW Code of Ethics and practice, ensuring ethical standards guide micro-level interventions within the larger systemic framework. Other additional duties as assigned. Qualifications BSW or MSW degree; with 5 years related experience, two of which must be working with street involved youth. Registration in good standing with the College of Social Workers and Social Service Workers. Strong commitment to integrated model of practice (client centered, AOP/ABR, trauma informed, harm reduction, Housing First for Youth, strengths/resiliency model based). Demonstrated expertise in case management and effective crisis intervention with youth who are experiencing complex challenges related to homelessness such as equity and access needs, harm reduction strategies, mental health support, in addition to their basic needs.
