**Position Description**:
The Patient Care Information Coordinator, Heart Transplant Program, will be responsible for patient scheduling processes and facilitating the delivery of patient care by providing a high level of support and organizational assistance to program staff and patient/families. They will support the operation of the team by providing clerical and administrative functions in a proactive and efficient manner. This role will be primarily responsible for all scheduling and coordinating activities for patients coming for heart transplant assessment and for post-transplant care. There is significant interaction across many departments at SickKids to coordinate care. The coordinator also supports clinic flow during clinic operation by ensuring smooth registration, patient rooming and service delivery. The patient care coordinator is the main point of contact for families in the outpatient setting.

**Here¿s What You¿ll Get To Do**
- Respond to requests for information from families and healthcare staff
- Ensure related scheduling requests are processed; reschedule appointments as necessary and provide updates to the clinical team
- Facilitate communication flow via telephone, paging, and computer access
- Maintain appointment schedules in Epic and notify patients/families of any updates or changes to their appointments; participate in clinical rounds as needed for the purposes of care coordination
- Greet and orient patients and their families to the program/clinic
- Maintain an efficient and tidy work environment
- complete training for external transplant database and maintain patient demographic records as required
- Act as the liaison for external hospital contacts

**Here¿s What You¿ll Need**
- Health-related post-secondary education (Medical Administration, Business Administration, Health Administration) preferred
- Minimum of 2 years clerical or administrative experience in a health care setting. Experience in a hospital environment is preferred
- Scheduling experience is essential and medical terminology is an asset
- Accurate keyboarding skills and strong working knowledge of Word, Excel, PowerPoint, and Outlook
- Experience with Epic is an asset
- Excellent problem solving skills that demonstrate an ability to anticipate potential issues and make suggestions
- Strong interpersonal, communication and organizational skills with both fellow colleagues and patients
- Ability to remain focused and prioritize tasks in a busy and high stress environment
- Dependability and good attendance

**Employment Type**

0.5 FTE, permanent employment; must be available to work Monday to Friday day shift with a requirement to work on Thursday for clinic
