On Site Support Specialist

**Overview**
Reporting to the On-Site Team Lead, the incumbent will be responsible for the delivery of on-site support services in our 24-hour program. In addition to fulfilling the Key Responsibilities and Accountabilities to the Organization, the On-Site Support Specialist’s will be responsible to provide support to individuals to achieve their rehabilitation and recovery goals, and to assist in reaching their full potential. This position may require flexibilityto work-days, evenings, and overnights (awake) on weekdays, weekends and on all public and general holidays. Daily schedule may vary according to program needs. This is a bilingual position in both official languages.
**Qualifications and Abilities**
- Degree, Diploma or Certification in a mental health and/addictions or social services discipline.
- 2-5 years’ experience in the provision of mental health and/addictions or social services.
- Understanding of mental health, substance use and addictions issues.
- Good critical thinking and problem-solving skills required.
- Acute sense of discretion, confidentiality and professionalism.
- Ability to function in a demanding and stressful environment
- Demonstrated ability to work with mínimal supervision.
- Strong written and verbal communications skills in English and French.
- Experience in computer programs i.e. Microsoft Office, Outlook.
- Satisfactory Employment record.
- Provide current and satisfactory Criminal Record Check (with _Vulnerable Persons Act_).
- Provide current and satisfactory Adult and Child Abuse Registry Checks.
- Applied Suicide Intervention Training Skills (ASISI)
- Non-Violent Crisis Intervention (NVCI ) training and certification
- CPR/Emergency First Aid (Level A)
- WHMIS certification and an understand the safety precautions and emergency procedures of potential hazards, including Personal Protective Equipment (PPE) use and standards.
**JOB DESCRIPTION**
**Rehabilitation and Recovery**
- Work with Service Recipient (and assigned staff) to determine individualized Rehabilitation and Recovery Plans, including SMART Goals and Action Plans focused on successful attainment.
- Communicating with service teams and attending case conferences as needed.
- Provide motivational support and assistance with time/daily schedule management through regular meetings, or as directed by Service Recipient’s needs.
- Support Service Recipient’s medication compliance, as determined by their Plans.
**Skill Development**
- Assist Service Recipients with improving and maintaining Instrumental Activities of Daily Living (IADLs) through instruction, role modeling and provide the skills and strategies necessary to successfully engage in their own mental health recovery process.
- Develop outcome measures and evaluation tools directly demonstrate program effectiveness.
- Monitor ongoing assessments to keep current, and approve changes made as required based on Service Recipient feedback, progress, and outcome measures.
- Assist in the acquisition of additional networks of support and services
- Medication Management and Education; understanding purpose and use of medication
- Support to maintain physical living environment as required.
- (Weekly in-suite housekeeping is provided by Manitoba Housing)
- Support and facilitate engagement with other care providers as required
- Facilitate basic life skills development and providing services that facilitate recovery
- Daily engagement and ongoing assessment in client’s unit to monitor for personal hygiene, safety concerns, visitors, damages, and pest, (including thorough bed bug checks weekly)
- Daily support preparing breakfast meal, using ingredient provided by Manitoba Housing.
- Daily support for assisting service recipients to attend and receive lunch and dinner provided by Manitoba Housing
- Assist in laundering services recipient’s clothing and linens as required in order to maintain personal and environmental hygiene (weekly linen laundering provided by Manitoba Housing)
- Preparing suites for pest management treatments (treatments provided by Manitoba Housing)
- Support stable residencies through provision direct services, engagement of other supports as required and engagement with Manitoba Housing.
- Ensure individuals meets their obligations under the participation agreement between the individual, WRHA and Manitoba Housing and Sara Riel to enable successful residencies
- Support will be provided on an ongoing basis if required to replenish, replace or obtain needed items with monthly stipend._**Tenancy Support Services**_
- Assist Service Recipients in understanding their tenancy responsibilities, connecting with the community, and navigating additional support services as needed.
- Support transitional services upon a level of achievement or upon the service recipients request - in preparing for, finding, getting and keeping housing in their community of choice.
