Youth in Transition Worker
Full-time, Term until March 31, 2026
It is the mission of Tungasuvvingat Inuit (TI) to broadly provide Inuit-specific programs that contribute to the health and wellbeing of Inuit and to encourage and support similar programs for Inuit across the country. The Youth In Transition Worker (Anti-Human Trafficking) is accountable for delivering culturally relevant and trauma
- informed support services to Inuit youth transitioning out of human trafficking situations or exploitation. This role involves developing individualized support plans, connecting youth with necessary resources, and promoting safety, healing, and well-being. The Youth in Transition Worker is responsible for maintaining confidentiality, respecting cultural practices, and adhering to program policies and procedures while ensuring that services are delivered with compassion, empathy, and a commitment to the values and principles of the Alluriarniq Program. The worker is also accountable for documenting all interactions and services provided, participating in ongoing training, and actively contributing to program development and evaluation.
**Key responsibilities of the position include**:
- Manage a caseload of active clients.
- Develop and maintain rapport, trust, and ethical relationships with clients in order to support them on their healing journey.
- Conduct comprehensive, age-appropriate assessments of needs and risks; develop collaborative safety, care, and goal plans that respect youth voice and choice while complying with legal obligations under Ontario law.
- Ensure youth clients are supported in understanding their rights to privacy, confidentiality, and consent, including the limits of confidentiality in cases involving duty to report under the Child, Youth and Family Services Act.
- Respect the unique legal status of youth aged 16-17 as outlined by the CYFSA, including voluntary youth service agreements and transition planning supports up to age 24.
- Facilitate safe and effective connections to services, resources and supports, including (but not limited to): Inuit Elders and community activities, legal services, health care, NIHB, social assistance, mental health and wellness services, addictions treatment, housing, parenting programs, pre
- and post-natal care, early years programs, food security, education and training, and employment services.
- Support reconnection with family and community where safe and desired, including youth-specific cultural and land-based programs and Land Claim Organization enrollment.
- Provide consistent emotional regulation support, psychoeducation, crisis intervention, and conflict resolution appropriate to youth development stages.
- Respond appropriately and in a timely manner to crisis situations; carry out crisis stabilization, service delivery, and client contact in a timely and professional manner.
- Participate in case planning and other CAS-related case conferencing; actively contribute to safety plans, service plans, care plans, and transition plans.
- Advocate for clients, encourage and support them to exercise their rights and responsibilities, and assist them in overcoming barriers to receiving the programs and services they need. Build capacity of clients to advocate for themselves to advance their self-determination and wellbeing.
- Offer advocacy and accompaniment for youth navigating systems such as the child welfare system, Ontario Court of Justice (youth division), health care, education, or indigenous services.
- Contribute and support the understanding of the importance of ongoing cultural learning and connection to community.
- Facilitate supportive networks of community who can assist individuals to achieve their goals and provide a safe and healthy environment.
- Integrate strength-based, culturally safe, ancestrally-affirming, decolonial and anti-oppressive practices in all service delivery.
- Ensure every youth care plan includes safety protocols, ongoing risk assessments, and consent renewal appropriate to evolving capacities and legal requirements.
- Conduct safety assessments and create safety plans tailored to each youth's needs, offering immediate crisis intervention when required and supporting youth in accessing emergency services, shelters, or safe housing.
- Advocate for youth to access culturally relevant, trauma-informed, and accessible services including transitional housing, education, harm reduction, and LGBTQIA+ affirming care.
