- Kennedy House Youth Shelter, for youth aged 16-24 experiencing homelessness.

**Position Summary**

Under direct supervision of the Program Manager, the Residential Clinical Coordinator is responsible for the planning, approval and implementation of on-site services that will provide educational, social, emotional, health, and recreational benefits to youth within all programs. The Residential Clinical Coordinator is to ensure programs are assisting residents to make the best use of available opportunities and resources, while fully developing each client’s individual potential. The Residential Clinical Coordinator provides all necessary liaisons with outside agency personnel, maintaining a high level of communication and positive public relationships.

Responsible for ensuring provision of service to the young people in care within the residential programs. The Residential Clinical Coordinator will ensure follow-up on formulated case plans and developed strategies of intervention. The Residential Clinical Coordinator position is designed to assist with the development and facilitation of individual and group programs. The Residential Clinical Coordinator is expected to take an active role in furthering offered programs to clients in an effort to raise the quality of service delivery.

**Duties and Responsibilities**
- To ensure program staff are meeting the needs of the youth in accordance with established case planning. To review Plan of Care and assist with implementation of the necessary strategies and services to meet the youth’s needs.
- To ensure serious occurrences is accounted for and properly reported as required
- Support with residential intakes and client admission for all programs
- Ensure or provide support and education to program staff to assist in their understanding of the client needs / issues.
- Provide individual and program support for the facilitation, implementation and maintenance of school programs.
- Provides for intake support on an as-needed basis including the updating of KHIMS, Filemaker and CARS
- Compilation and distribution of required residential statistics (i.e. occupancy, quarterly, days of care, etc).
- Compiles required data and forwards to financial department to assist in monthly invoicing process and follow up when necessary (i.e. Special Billing, Per Diem, Medication, Dental, etc.).
- Ensures serious occurrence reporting and PACY protocols, are complied with.
- Completes documentation for DS programs as needed ie. Plan of cares, discharges, ISP...
- Ensure compliance with the collective agreement.
- Ensure or provide support to the youth / staff in an attempt to assist in their understanding and coping with the transition to a Residential Placement.
- Collaborate with the school worker’s and teachers to coordinate programming and services with the education system.
- To identify client service concerns, alerting the Program Manager.
- To complete administrative requirements in accordance with program responsibilities / requirements.
- Ensure program support by presenting as a resource to residential supervisors, offering advice, providing guidance and giving direction with regards to decision making and requests for approval.
- To provide information and support, as necessary, to the appropriate parties in Serious Occurrence Investigations.
- To ensure program support and Supervisory coverage during absences by providing back-up coverage for team members when assigned.
- Attend all program / client meetings as assigned.
- To work a flexible schedule, predetermined by the Program Manager, in accordance with program requirements and client need.
