At Comfort Keepers, we provide uplifting in-home care for seniors and adults who require a helping hand, a supportive companion, and day-to-day assistance in their home. As the Client Care Coordinator, you will work with a network of caring professionals that helps Comfort Keepers deliver one-on-one care that enhances the quality of life of our clients and their families. As a valued member of our Care team, you will assist clients and their families in their home care journey by determining their needs and ensuring they are provided with the care they require, where and when they need it. This is a hybrid position where you will work from home some days and will attend our North York (Don Mills/Sheppard) office location some days combined with time spent in the community visiting clients.

**We are currently recruiting for a full-time Client Care Coordinator. The schedule for the role is Monday to Friday 8:30 am to 4:30 pm.**

**What will you do?**
- In collaboration with patients and their families, assess their care needs, and develop individualized care plans, coordinate, and monitor care plan delivery
- Work in tandem with the scheduling department and front-line staff to ensure client needs are met.
- Explore business development opportunities and network with referral sources within the community
- Identify and resolve potential client issues that may arise

**What must you have?**
- Excellent interpersonal, communication, assessment, and decision-making skills
- A strong sense of empathy, compassion, problem solving and customer service
- Effective time management skills, with the ability to work independently and co-operatively in a busy environment
- A valid driver’s license and access to a reliable vehicle
- Strong computer literacy, including proficiency in the Microsoft office suite

**What would give you the edge?**
- Experience working with diverse patient groups, e.g., multicultural, acquired brain injury (ABI), mental health, geriatrics, dementia, Alzheimer’s, etc.
- Ability to speak a second language is an asset
- Being “tech savvy” and a quick learner with technology
- Customer service and/or intake experience

**Compensation?**

50-55k per annum plus quarterly bonuses.
