Position Summary

The Program Manager - Homecare Modernization & HISH provides an integrated patient-centred approach to care that focuses on enhancing and coordinating the care for patients living within, or part of the Couchiching Ontario Health Team catchment area. This Program Manager works with all system stakeholders to plan, transition and implement homecare modernization initiatives that align with the transition of homecare into the Couchiching Ontario Health Team (COHT).
The role of the Program Manager is to create seamless, integrated and modernized care for patients needing homecare in Couchiching. The Program Manager will ensure that each patient has a Coordinated Care Plan (CCP) and ongoing care coordination that is done in a fully integrated, patient and caregiver focused manner. The Program Manager is responsible for working with system stakeholders on extensive project planning, stakeholder engagement and modernized transition initiatives, that will transform homecare for Couchiching, in alignment with Provincial and local direction.
This role works as an integral part of the COHT operations and planning towards integrated care and collaboration for enhanced patient care planning. Further, this position fosters partnerships with internal and external stakeholders that promote safe, effective, evidence-based and quality outcomes in a patient-focused environment.
**PRIMARY RESPONSIBILITIES**:
Contributing to maintenance of relationships/partnerships through:
- Establishing, developing and maintaining collaborative, collegial and interdisciplinary partnerships with all internal/external stakeholders to facilitate integrated and seamless care for patients across care partners.
- Utilizing effective problem-solving options/alternatives to achieve best possible outcomes for all involved.
- Facilitating partnerships to: define the needs of the program, identify needs of the populations served, problem solve common issues and formulate action plans to enhance the delivery of quality care.
Ensuring quality communication throughout Integrated Care Services and partner services by:
- Meeting regularly with the Director/ Medical Director, COHT, Managers and direct care providers within the Program and Community
- Creating and maintaining effective communication processes with direct reports, between Programs, interdisciplinary Program team(s), the patient, the family and the community.
- Evaluating the effectiveness of communication systems and mechanisms
- Fostering an environment that enables communication, modernized thinking and participation by modeling approachability, visibility and effective written/ verbal interpersonal skills.
Ensuring quality resource management and measurement of Homecare Services through:
- Conducting regular quality improvement reviews, identifying performance deficits in collaboration with the COHT Performance committee
- Maintain staffing appropriate for the Program in collaboration with system stakeholders or contracted providers
- Scheduling staff, allocating leaves of absence, vacation schedule planning and authorizing overtime consistent with the operational needs of the unit(s) and within the scope of contractual requirements.
- Daily fiscal and operational management of the program inclusive of risk management, workload analysis, participation in patient care rounds (as applicable), communication with direct reports/patients/families, provision of quality care and conflict resolution.
- Communicating/reporting to the Program Director(s) on a regular basis.
- Identifying educational needs of the staff/unit and collaboratively planning strategies with the COHT
- Conducting regular unit/service meetings to ensure all direct reports have opportunity to receive or exchange information.
**In conjunction with the Program Directos, responsible for**:
- Evaluating human resource needs collaboratively with Human Resources, and direct care providers to select, retain and recruit individuals to meet the needs of the Program.
