**CLIENT CASE WORKER**

This position is new to the organization and is designed to work with First Nation(s) residing in Prince Albert Urban Geographical area.

**Responsibilities**:
Under the direct supervision of the PAGC Urban Services Director, the Client Case Worker will have the following responsibilities:

- Undertake client case management activities for employment success, according to the SITAG process, with clients who reside in the city of Prince Albert and PA Urban Geographical area.
- Perform initial assessment with clients
- Assist clients in obtaining employment and career information
- Facilitate and coach clients to identify a personal career destination and develop an action plan
- Refer clients to appropriate services and/or organizations
- Prepare client files for sponsorship
- Monitor clients progress in sponsored activities and provide support to them when required
- Track clients’ employment status, periodically and offer job maintenance/retention support
- Maintain computer-based records to keep track of clients’ participation in the case management process and their funding requirement when applicable, provision of reports when required.
- Other functions as may be assigned from time to time.

**Qualifications**:

- Must possess a diploma from an accredited institution in human services or administrative discipline A combination of life experience, work experience and education
- Experience in working with First Nations clients
- Effective communication and interpersonal skills
- Possess a valid driver’s license and the ability to travel in the surrounding area of Prince Albert.
