**Job Summary**
As part of a dynamic provincial organization, the Regional Services Co-ordinator provides coordination of services, active support, resources, and education. The position focuses on client centred goals and solutions to navigate system barriers that prevent persons with spinal cord injuries living the life they choose.
This Regional Services Co-ordinator position requires an innovative and resourceful individual to assist with serving clients in **London and surrounding areas** (e.g., Windsor). This full-time position is based out of a home office.
**_Reporting to the Manager, Service Co-ordination & Intake, you will: _**
- Work with individuals with a spinal cord injury and their families in acute care, rehabilitation and in the community
- Act as a resource, navigator, advisor and facilitator, supporting and advocating for clients, their family and the community
- Work with clients on goal setting and attainment in the areas of equipment, housing, transportation, health and wellness, recreation, personal support services, and funding.
- Provide information and education on a wide variety of topics related to spinal cord injury
**_Qualifications: _**
- University Degree in rehabilitation science, social work, social sciences or equivalent education and experience
- Minimum three years’ experience in a rehabilitation setting within community services as well as other social and health related disciplines or similar related experience
- Holistic/generalist knowledge of mobility disabilities and their functional and emotional implications
- Proven assessment skills, conflict resolution and mediation experience
- Excellent verbal and written communication skills
- Solid planning and time management skills
- Comprehensive knowledge of community service and support networks
- Experience with case management databases; knowledge of interRAI Community Health
Assessment, an asset
- Demonstrated computer proficiency working in an MS Office environment
- Ability to travel within the region, with occasional provincial travel.
