**ABOUT THE ROLE**
The Service Coordinator, Urgent Response is a full-time permanent position that provides service navigation, planning and support to families with infants, children and youth with mental health concerns, autism and developmental disabilities from birth to 18 years, who are experiencing complex emotional, psychological and behavioural concerns and crisis to enable timely connections to needed services.
**_ Salary range:_** _$55,750 to $64,921 per annum_

**WHAT YOU WILL DO**
Connect and collaborate with children, youth, and families who are experiencing high risk emotional, psychological and behavioural concerns and develop a working plan to support their complex needs.
- Initiate contact to needed services for child/youth and family according to the service plan, provide support to child/youth and family experiencing crisis using approaches that best meet the clients’ needs.
- Outreach about services and connect with potential community resource providers about their services, referral processes, eligibility criteria, wait lists, and other details to be informed for making future referrals.
- Ensure accurate and timely documentation in client information system(s) in accordance with Ministry guidelines, organizational, and accreditation standards.
- Support to families evidence-based tools, including the principles of Applied Behaviour Analysis (ABA) and Cognitive Behavior Therapy to support the children, youth and parents in managing and reducing at risk behaviours.
**THE “MUST HAVES”**
The following education, professional memberships and knowledge are a requirement for this role:
- Bachelor’s degree in either Social/ Health Sciences, or Bachelor in CYW or Bachelor in Social Work.
- 1 year of related experience working with children and youth with autism spectrum disorder / developmental disabilities and their families, with a focus on community engagement.
- Membership in the appropriate professional regulatory body.
- Awareness of the role of diversity, equity, and inclusion in the workplace and within the community being served by the position.
- An understanding of anti-black racism, anti-oppressive/anti-racism perspective in the context of the social determinants of health.
- Ability to recognize and identify situations where incidents of imbalance of power and privilege may occur and can result in possible negative impacts.
**THE ‘NICE TO HAVES’**
- Experience on an interdisciplinary team and with multicultural environments is an asset.
- Functional computer and client information system knowledge.
- Fluency in another language(s), including sign language.
