Village Family Health Team serves the rapidly growing neighbourhoods of Liberty Village, King/Queen West and Parkdale in Southwest Toronto. **Position Summary**:
The Manager of Programs and Services plays a key leadership role in the planning, development, implementation, and evaluation of interprofessional programs and services at VFHT. This role supports the integration of best practices and ensures programs are responsive to the needs of our patients and aligned with organizational goals and provincial health priorities.

**Key Responsibilities**:

- Lead the design, implementation, and evaluation of team-based programs and services, including chronic disease management, mental health and addictions, health promotion, and population health initiatives
- Ensure programs are patient-centered, evidence-informed, and culturally responsive
- Develop and monitor work plans, outcome indicators, and quality improvement goals for all programs
- Provide direct supervision to interprofessional staff (e.g., social workers, dietitians, nurses, pharmacists, health promoters)
- Support recruitment, onboarding, performance development, and professional growth
- Foster an inclusive, collaborative, and accountable team culture
- Collaborate with the Executive Director and Leadership Team to align programs with strategic priorities, funding requirements, and health system developments
- Analyze service utilization data and community health needs to inform planning and resource allocation
- Lead or support QI initiatives related to clinical and non-clinical programs
- Monitor key performance indicators and contribute to reporting requirements for funders
- Promote patient and community engagement in service design and improvement

**Qualifications**:

- Degree in a health-related or social service field (e.g., nursing, social work, health administration, public health)
- Master’s degree preferred
- Minimum of 5 years of progressive experience in health care or community services, including supervisory or management experience
- Demonstrated experience in program development, team leadership, and quality improvement
- Knowledge of primary care, health equity, and Ontario’s health system reforms (i.e., Ontario Health Teams)
- Excellent interpersonal, communication, and organizational skills
- Ability to think strategically, solve problems, and manage multiple priorities
- Experience working with electronic medical records (VFHT uses Accuro EMR), data systems, and evaluation tools is an asset

**Reporting Structure**:
The Manager of Programs and Services will report to the Executive Director for day-to-day operations.

**Job Specifications**:
Location: 171 East Liberty St., Unit 102.

Hours of Work: 37.5 hours per week, generally aligning with FHT practice operations, with some evening and Saturday as necessary.

Status: 1.0 FTE, full-time, permanent.
