This position will report directly to SAHS’ Program Supervisor and is very team focused.

**MAIN PURPOSE**: To engage service provider and partners attached to the homelessness-support system and to facilitate the Coordinated Housing Access Team (CHAT) while sustaining Whitehorse’s By-Name List and coordinated access initiatives. This position will work with Built for Zero-Canada and works closely with the Homelessness Individual and Family Information System (HIFIS) Project Manager & Data Lead at SAHS.
**POSITION QUALIFICATIONS AND SCOPE**
**Responsibilities**:
- Coordinate the on-going improvement of Coordinated Access (CA), a community-based systems approach to ending homelessness with partners, including the Reaching Home Community Advisory Board, Indigenous organizations and governments, and the Community Entity for Yukon - the Yukon Anti-Poverty Coalition.
- Broadly promote the CA system in Whitehorse and potentially, in selected Yukon communities with public systems, service providers, First Nations, the public and individuals with lived experience of homelessness.
- Facilitate engagement and create dialogue amongst community homeless-serving system partners in the planning and implementation of CA on a regular basis.
- Evaluate existing community approaches to CA and homelessness response, identifying strategies and approaches that reflect best-practices and that are tailored to evolving and changing community needs.
- Work closely with the Data Lead to validate the quality of the By-Name List of individuals experiencing homelessness and work with community partners to fill housing vacancies.
- Coordinate intakes onto the By-Name List and support common assessment, referral and prioritization of individuals and families experiencing homelessness across the system.
- Ensure that confidentiality and all operations related to CA are done in accordance with relevant privacy legislation (i.e. ATIPP and HIPMA).
- Monitor Whitehorse’s Coordinated Access Process Guide on behalf of the Coordinated Housing Access Team in consultation with key stakeholders.
- Lead capacity building within the housing and homelessness service sector, coordinating learning and training opportunities for service providers across sectors.
- Ensure system integrity and alignment with evidence-based practices to provide accountability to all levels of government for funding and service delivery standards.
- Engage and support landlords and housing providers as needed, with a particular focus on supporting access to new units attached to the By-Name List.
Qualifications:
- Completed post-secondary education in social services or related field and a minimum 2 years work experience related to community development, project management, and/or housing and homelessness supports;
- Knowledge of decolonizing and housing first approaches to client support;
- Awareness of the complexities of homelessness, poverty, and the colonial context of homelessness in Yukon, and related issues;
- Strong understanding of community agencies, resources, and the cross section of service providers in Yukon; and
- Knowledge of SAHS vision, Built for Zero Canada, the Canadian Alliance to End Homelessness’ allied networks, Reaching Home: Canada’s Homelessness Strategy, and related plans/initiatives.
Skills & Abilities
- Computer skills with proficiency in Microsoft Office and Excel;
- Knowledge of the Residential Landlord Tenant Act an asset;
- Demonstrated ability to work independently and as part of a team;
- Strong written and verbal communication skills for report writing and facilitating presentations to large and small groups;
- Possess the ability to manage competing priorities and coordinate several tasks at one time.
