Full Time Cognitive Support Lead Psw

The IOOF has recently embarked on the largest capital project in the IOOF’s history, with the launch of a $1.2 million redevelopment campaign to expand the facility to a 226-bed long-term care home. This transformative revitalization will create 64 new long-term care beds and 66 upgraded beds, modern spaces, updated equipment, private rooms, and increased square footage.

As our facility embarks upon this exciting expansion, our team is growing too We are adding RNs, RPNs, and PSWs.

**Cognitive Support Lead-Personal Support Worker (PSW)**:
The Cognitive Support Lead -Personal Support Worker (PSW) is responsible for providing resident care to residents under the direction of the Cognitive Support Lead-RPN and Management. The cognitive Support Lead-PSW participates in the NSM Specialized Geriatric Services model within the LTC Home, enhancing services for residents with behaviours/expressions associated with complex and challenging mental health, dementia, substance use and/or other neurological conditions. The Cognitive Support Lead-PSW demonstrates a person-centered approach to care and participates in the assessment, planning, implementation and evaluation of behavioural interventions for residents within the interdisciplinary team. The Cognitive Support Lead-PSW participates in education and knowledge transfer to all staff related to expressions/behaviour management.

**Qualifications**:

- Graduate of an approved Personal Support Worker program
- Prior experience with clients in long term care, community and/or other Mental Health facilities.
- Prior knowledge and experience working with individuals with various types of dementia, delirium, mental health conditions, and who have experienced an acute behavioural change.
- Training, or the ability to obtain training in P.I.E.C.E.S., Gentle Persuasive Approach, U-First, Montessori, Non Crisis Intervention etc.
- Certificate in Dementia Care considered an asset.
- Basic principles of providing education to adults considered an asset.

**EDUCATION & EXPERIENCE**:

- Possess a Personal Support Worker Certificate or equivalent as per O. Reg. 79/10 of the Fixing Long Term Care Act, 2021;
- Two (2) years' related experience working in a long-term care facility - preferred;
- Proficient working knowledge of computers and computer literacy
- required;
- Advanced skills in the use of Point of Care (POC) preferred; and
- Additional studies completed in Activation Techniques, Gerontology, First Aid and/or CPR - preferred.

**KNOWLEDGE & ABILITIES**:

- The applicant must have proficient knowledge in the following areas:_
- Working knowledge of the _Ministry of Health and Fixing Long Term Care Act, 2021, _and related Ontario Regulations;
- Familiarity with Compliance under the LTCHA - the LTC Quality Inspection Program and the Inspection Protocols (IP’s) for Resident Care;
- Good understanding of Continuous Quality Management (CQI) principles, legislative requirements for nursing services;
- Computer literacy including computerized Resident charting;
- Ability to work with and relate to interdisciplinary team members, Management and other internal/external stakeholders;
- Ability to effectively prioritize and deal effectively with competing priorities
