The Belleville Nurse Practitioner-Led Clinic (BNPLC) is seeking a Social Worker to join our interdisciplinary health care team. Since 2010, the BNPLC has been a trusted leader in providing accessible, individualized, fully responsive primary health care in the Quinte Region. With more than 3500 registered patients with a diverse range of needs, we make a difference every day.
**Position Summary**
As a member of the Belleville Nurse Practitioner-Led Clinic, the Social Worker will work collaboratively with the primary healthcare team to provide patient-centered, quality primary health care to our patients.
The Social Worker provides psychosocial assessment, counselling, case management and health promotion to individuals, couples, families and groups.
The Social Worker will build relationships with community organizations to facilitate linkages to appropriate services and resources.
**Duties and Responsibilities**
- Provide mental health counselling and services to individuals, groups, couples and families
- Provide primary mental health care for clients with mild to moderate mental health illness
- Collaborate with the interdisciplinary health team to support client care and needs
- Implement individualized care plans for the client based on best practice guidelines and evaluate/assess achievement of client goals.
- Facilitate coordinated access to community services in areas such as social determinants of health, assistance with daily living, crisis intervention, and mental health care
- Advocate on client's behalf as needed
- Participate in committees or community groups where an identified need for advocacy or representation is required
- Design, implement and evaluate health care programs for the BNPLC in coordination with all members of the health care team; that address current gaps in service, target population or population health
- Provide culturally sensitive care
- Submit statistics as required by Ontario Health and/or BNPLC
- Participate in training sessions as required by the BNPLC
- Participate in regular team meetings, and other meetings as requested
- Facilitate group educational sessions and activities using principles of child, youth, adult or older adult learning.
- Assists in the maintenance of a clean, orderly work environment
- Maintain confidentiality in accordance with the Personal Health Information Protection Act (PHIPA, 2004) and all other relevant legislation.
- Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the BNPLC.
- Other duties as required
**Skills/Knowledge Requirements**:
- Demonstrated ability to work effectively independently and as a member of an interdisciplinary professional health team, with excellent interpersonal and communication skills
- Exceptional documentation skills and attention to detail
- A positive and professional attitude
- Excellent time management skills with an ability to multi-task and effectively set priorities
- Knowledge in current, evidence-based therapeutic modalities
- Proficient in use of internet and Microsoft software
- Experience with an Electronic Medical Record, e.g., Telus EMR
- Experience in program design, delivery and evaluation an asset
**Education/Specific Job Requirements**:
- Graduate degree in Social Work from a recognized university.
- Must possess a minimum of 2-5 years of experience in a primary care or similar practice setting;
- Three to five years counseling/therapy experience: preferable in a community or primary care setting
- Current registration with Ontario College of Social Workers and Social Service Workers and evidence of Professional Liability Insurance coverage
- Proficiencies in assessment, motivational interviewing, psychotherapy modalities for working with individuals, couples, families and groups
- Demonstrated knowledge of the Mental Health Act
- Addictions counselling experience would be an asset
- Excellence in problem-solving and conflict resolution
- Knowledge of community resources and experience in developing community linkages and relationships
- Certification in CPR
- Ability to travel throughout the community
- Able to work flexible hours including evenings and Saturdays
- Have at least two Covid-19 vaccinations or a valid medical exemption
- Current Police Vulnerable Sector Check (less than 6 months old)
**Physical Demands**:
- Majority of time will be spent with clients providing mental health support in an assigned office
- Moderate: The position requires mínimal physical effort, and the workday comprises roughly equal amounts of standing and sitting, with frequent position changes.
Pay: $35.00-$36.00 per hour
Expected hours: 40 per week
