**Job Overview**: Reporting to the Director of Primary Health Care, the Panel Manager will ensure that the clinics are utilizing panel identification and maintenance activities that comply with provincial standards. The Panel Manager will assist The Alex with full participation in the Community Information Integration and Central Patient Attachment Registry (CII/CPAR) and will ensure all quality assurance practices for panel management are in place. This is a permanent, full-time position.

**Key Responsibilities**:

- Conduct panel identification and maintenance activities including patient statuses and verification processes.
- Ensure each patient is assigned a primary care provider (PCP).
- Resolve unassigned patients by reviewing appointment history to determine appropriate assignment based on PCP availability.
- Monitor influx of new patients into panels and analyze PCP capacity (panel fullness) to absorb new patients
- Track PCP status changes (transfer, termination, sabbatical, medical leave, etc.) and in collaboration with clinic managers:

- Evaluate impact on assigned panel of patients.
- Reassign panel to other clinic providers according to panel fullness.
- Notify affected patients of new PCP assignment.
- Document clinic processes pertaining to panel identification and maintenance.
- Manage and support training of clinic teams who assist in panel work.
- Work with the Evaluation and Impact team to develop monthly panel reports and review PCP assignments.
- Measure, document and track other relevant metrics such as Third Next Available appointment, Panel Sizes, and Continuity while engaging and supporting clinic staff to embed these activities in clinic workflow
- Assist physicians and clinic staff with EMR standardization, including document keywords.
- Oversee clinic teams with review and regular touch points of patient panel lists.
- Support preventive screening and treatment adherence in primary care, ensuring that each patient encounter is maximized and used appropriately for preventive and basic care.
- Work in collaboration with Primary Care Network Panel Managers to resolve panel conflicts.
- Participate in presentations as needed.
- Perform other duties as assigned.

**Qualifications and Experience**:

- Minimum current Medical Office Assistant Certification or other relevant post-secondary education preferred.
- A minimum of three years in a primary care environment demonstrating expertise in at least one
- Advanced knowledge and experience with electronic medical records, including reporting and registry functions.
- Strong understanding of primary care clinic operations and the ability to support clinic staff in learning new skills
- Knowledge of quality improvement principles and tools.
- Strong data and analytical skills.
- Proficient with Microsoft Office suite or related software
- Excellent communication stills
- Ability to build strong working relationships and assists in fostering collaboration and teamwork
- EMR platform (Wolf, PS Suite, MedAccess or CHR)
- A satisfactory vulnerable sector criminal record clearance and child welfare intervention check are required.
- Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action.

**Work Environment and Hours**:

- Community Health Centre (102, 2840-2 Ave SE, Calgary)
- Standard Hours: Between 0830 - 1800; 8 hours shifts
- 1.0 FTE
- Continuous extended periods of sitting, with some standing and walking (sometimes up/down stairwells)
