Sommaire des responsabilités Polyvalence entre Montfort, l'unité de transition Beacon Heights et Carrefour Santé Aline ChrétienSous la supervision et en collaboration avec la Gestionnaire clinique, le titulaire agit à titre de professionnel en soins de santé primaires et est responsable des soins physiothérapeutiques des patients de l’Hôpital Montfort. Les responsabilités clés sont :•Évaluer la fonction et la mobilité du patient, élaborer des buts de traitements et d’établir un plan de traitement centré sur le patient et en fonction d’une pratique basée sur l’expérience et les preuves scientifiques;• Participer activement à la planification de congé du patient et le diriger vers d’autres services, lorsque nécessaire;• Documenter les interventions au dossier du patient selon les normes du collège des physiothérapeutes de l’Ontario;• Participer à la gestion des équipements du département;• Participer aux différentes tâches administratives déléguées par le Gestionnaire clinique des services thérapeutiques;• Maintenir des statistiques à jour selon le système de mesure de la charge de travail;• Informer un remplaçant de sa charge de travail et toutes informations pertinentes concernant le plan de soins des patients par biais du tableau de bord et des feuilles de route;• Organiser, coordonner et enseigner des sessions de formation selon les besoins;• Respecter les politiques et procédures pertinentes de l’organisation.Notez que : Les physiothérapeutes de l’Hôpital Montfort font des rotations d’unités de soins à tous les six (6) mois, à moins d’avis contraire selon le poste. Profil d'exigences • Détenir un permis de pratique valide de l’Ordre des physiothérapeutes de l’Ontario.• Détenir un certificat en réanimation cardiorespiratoire (RCR).• Posséder un minimum d’un (3) trois mois d’expérience en physiothérapie, préférablement en soins aigus.• Avoir complété des stages cliniques dans un milieu hospitalier.• Connaissance et habiletés informatiques – courriel, traitement de texte, tableaux de données.• Maîtriser les deux langues officielles (français et anglais), à l’oral et à l’écrit, à un niveau intermédiaire. Atout • Être membre ou être admissible à devenir membre de l’Association des physiothérapeutes du Canada.
