Job Title: Healing Plan Coordinator

Job Summary:

The Indigenous Women's Healing Centre is seeking a skilled Healing Plan Coordinator to join our team. As a key member of our team, you will provide case management activities for residents/offenders, completing follow-ups of Correctional Plans/Healing Plans. You will work with residents/offenders to confront and overcome conditions that have contributed to personal difficulties and conflict with the justice system, facilitating self-awareness, self-worth, and self-determination within the Indigenous context.

Key Responsibilities:

  • Develop and implement case management plans to support residents/offenders in their reintegration into the community.
  • Conduct regular follow-ups and assessments to monitor progress and identify areas for improvement.
  • Collaborate with residents/offenders to develop life skills and promote community involvement.
  • Prepare reports and maintain accurate records of case management activities.
  • Work effectively with Indigenous communities and organizations to provide culturally relevant support services.
  • Develop and maintain relationships with community partners to access resources and services for residents/offenders.
  • Participate in training and professional development opportunities to enhance skills and knowledge.

Qualifications:

  • Degree in Social Services/Criminal Justice or related field.
  • At least 2 years' experience in case management or a related field.
  • Knowledge of Indigenous issues and culturally relevant approaches and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Valid Class 5 Manitoba Driver's license.
