Job Summary
We are seeking a highly motivated and experienced Care Planning Specialist to join our team at Ma Mawi Wi Chi Itata Centre Inc. This is a full-time position, working 35 hours per week, with a competitive salary range of $65,000 - $85,000 per year, depending on experience.
Responsibilities
As a Care Planning Specialist, you will be responsible for providing holistic support services to our program participants, including those who are homeless or at risk of homelessness. Your duties will include:
- Providing individualized support services to participants, including home visits and developing plans to help them achieve their goals;
- Assisting participants in acquiring and maintaining a home, accessing emergency housing if needed, and providing knowledge and skills to become a good neighbor and successful tenant;
- Supporting participants' individualized plans of action and work on implementation of goals;
- Responding to participants' crisis situations in an immediate manner, assessing their needs, and delivering service in a timely, professional, and effective way;
- Liaising and consulting with collateral agencies regarding specific service recipients or general service concerns;
- Working cooperatively with others as a team member and contributing to a positive work environment;
- Ability to work under stressful situations;
- Strong oral and written communication skills;
- Understanding of Indigenous values;
- Valid Class 5 Driver's License;
- Current within three months Police/Child Abuse checks.
Qualifications
To be successful in this role, you will require:
- Youth Care Diploma or acceptable equivalent and related experience;
- Care planning and healing plan development and implementation skills;
- Working knowledge of crisis intervention strategies;
- Demonstrated experience in developing and maintaining effective working relationships with Indigenous participants;
- Liaises and consults with collateral agencies regarding specific service recipients or general service concerns;
- Needs to have ability to work cooperatively with others and contribute as a team member;
- Ability to work under stressful situations;
- Strong oral and written communication skills;
- Understanding of Indigenous values;
- Valid Class 5 Drivers License;
- Provide current within the last three months Police/Child Abuse checks.
