Mental Health Access and Discharge Coordinator

We are currently seeking a skilled Access and Discharge Coordinator to join our team at the Provincial Neuropsychiatry Program at UBC Hospital in Vancouver, BC.
About the RoleThis full-time position involves coordinating and implementing a centralized referral and access utilization system, developing and determining the effectiveness of access and discharge policies and procedures.
You will work closely with healthcare teams, service providers, and patients to ensure seamless transitions and effective communication.
Key Responsibilities- Develop and maintain bed occupancy and wait list databases
- Liaise with service providers and healthcare teams to facilitate referrals and access
- Participate in rounds, collect data, and review referrals
- Contribute to quality improvement activities and promote awareness of mental health programs
To be successful in this role, you will require:
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Two years' recent, related nursing experience relevant to the designated client population or an equivalent combination of education, training, and experience
- Valid BC Driver's License

The estimated salary range for this position is CAD $49.20/Hr. - CAD $63.47/Hr., commensurate with experience.
