Job Summary

Vancouver Coastal Health is seeking a Registered Nurse or Registered Psychiatric Nurse to join the Adult Mental Health team as a Discharge Follow-up Nurse in North Vancouver, BC.

The successful candidate will be responsible for monitoring and checking in with patients following a discharge from an acute Mental Health and/or Substance use stay in the hospital.

Key Responsibilities:

  • Ensure that the client is aware of discharge plans, community supports and conducts a point of care risk assessment.
  • Work in collaboration with the treating team to ensure the patient is aware of supports available to them in community.
  • Develop collaborative client care plans and refers clients to appropriate internal, community and/or professional services, as required.

Qualifications:

  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years' recent related clinical nursing experience in a community setting working with a variety of mental health & substance use clients, or an equivalent combination of education, training and experience.
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes, as required.
