At Traverse Independence, we offer a comprehensive range of supports and services to empower individuals with acquired brain injuries (ABI) to live independently in their communities.

This inspiring role is part of our outreach program, which focuses on building trusted relationships with clients and supporting them in achieving their goals, improving their quality of life, and coping with daily activities.

You will report to an Outreach Coordinator and work as part of a multidisciplinary team that includes regulated staff such as ABI specialized psychiatrists, occupational therapists, behaviour therapists, and intensive case coordinators.

Your responsibilities will include providing support to individuals in their homes or community settings across the Region of Waterloo. Experience working with people with ABI is beneficial but not necessary, as we offer a comprehensive training program both internally and through the Ontario Brain Injury Association.

To succeed in this role, you will need proven organizational skills, interpersonal skills, communication skills (both written and oral), and the ability to work collaboratively as a member of a multi-disciplinary team. You should also have a comprehensive understanding of Microsoft Office products.

We are seeking a candidate who holds a Community College Diploma in Social Services, Developmental Services, Health Care, or a related discipline. Demonstrated experience in behaviour management skills is advantageous, as is experience working with people with disabilities such as mental health and/or addictions. Strong utilization and understanding of M365 - Teams/SharePoint are also essential.

A valid driver's license and access to a reliable vehicle with proof of $1,000,000 liability insurance and a driver's abstract acceptable to Traverse Independence are required.
