Key Responsibilities:
- Deliver high-quality, patient-centered care to individuals and families in our community.
- Work collaboratively with other healthcare professionals to ensure seamless care coordination and continuum of patient care.
- Participate in quality improvement and evaluation activities to enhance patient outcomes and healthcare services.
- Utilize electronic medical records to document patient information and communicate with other healthcare providers.
- Develop and maintain strong relationships with patients, families, and community partners to promote health and well-being.
Requirements:
- Eligibility for licensure with the College of Physicians and Surgeons of Nova Scotia.
- Eligibility for certification with the College of Family Physicians of Canada.
- Membership with the Canadian Medical Protective Association.
