Job Title: Community Outreach Worker - Community Wellness

Job Summary:

The Community Outreach Worker will work proactively with new and existing clients, offering a range of support and recovery services, within an existing service plan and on an as-needed basis. The successful candidate will be a relationship-builder and advocate, providing services in a non-judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals.

Key Responsibilities:

  • Provide outreach services to hard-to-reach populations
  • Build and maintain relationships with clients and community stakeholders
  • Develop and implement individualized service plans
  • Provide support and guidance to clients in achieving their goals
  • Collaborate with other service providers to ensure comprehensive support

Requirements:

  • Minimum of 2 years' experience working in community, preferably with the program's target population
  • Experience providing outreach with hard-to-reach populations
  • Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
  • Commitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission, and values
