Licensed Practical Nurse, Outreach Specialist

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our multidisciplinary mental health services team in a collaborative nursing model with RNs and RPNs. The successful candidate will work as a member of the team to provide personal and nursing care services to clients with a variety of mental health disorders.
Key Responsibilities- Administer medications in accordance with pharmaceutical and organizational policies and procedures.
- Assess and monitor clients' actual and potential strengths and limitations, including physiological, psychological, socio-cultural, and spiritual needs.
- Provide a supportive environment for clients and contribute to the development and modification of client care plans.
- Plan, organize, and establish client care plan priorities when providing care, responding to unexpected events and changing client needs within scope of practice and competency guidelines.
- Provide nursing care to clients in their homes and in the community, according to identified policies and procedures, technology, and Fraser Health standards.
- Document and update relevant care and procedures information to ensure patient safety.
- Assist clients with activities of daily living, such as serving meals and feeding, client transports, bathing, dressing, skin care, toileting needs, and oversee client exercise routines as applicable.
- Participate in quality improvement and program activities to maintain a consistent level of care and ensure interdisciplinary team members work collaboratively.
- Maintain a safe working environment for patients and others by adhering to established safety policies and reporting unsafe conditions and/or malfunctioning equipment.
- Assist in the orientation and education of new staff, students, and other members of the interdisciplinary team, in collaboration with the Manager or designate.
- Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training, and experience.
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Current First Aid, CPR, and Food Safe certificates.
- Completion of or eligible to complete MHSU Competency Education.
- Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
