We are seeking a skilled and compassionate Continuing Care Assistant to join our team at Shannex's Transitional Care Centre in West Bedford, Nova Scotia.
As a Continuing Care Assistant, you will play a vital role in providing personalized care and support to patients who require additional time, services, and assistance to transition back to their previous living situation.
Key Responsibilities:
- Provide personal care and assistance with activities of daily living, such as bathing, dressing, oral care, skin care, meals, and mobilizing, according to the patient's care plan.
- Collaborate with the patient and other members of the interdisciplinary team in the development of individualized patient care plans.
- Assist in the planning and implementation of patient leisure activities on a daily basis.
- Assist with light housekeeping duties.
- Perform delegated nursing acts according to company policy and scope of practice.
- Communicate and report relevant information regarding patient care or safety to Supervisor.
Requirements:
- Valid Continuing Care Certification or Provincial Personal Care Worker course or equivalent.
- Registered in Nova Scotia's CCA registry as per the Continuing Care Assistants Registry Act.
- Ability to work a schedule of flexible hours and shifts inclusive of days, nights, weekends, and holidays.
- Clear criminal record with vulnerable sector screening.
- Alzheimer's and Dementia Care course considered an asset.
- Previous working experience with elderly in long-term care or a senior living environment an asset.
- Valid CPR & First Aid Certification preferred.
