Job Title: Continuing Care Assistant

Closing the Gap Healthcare Group is seeking a dedicated and compassionate Continuing Care Assistant to join our team. As a Continuing Care Assistant, you will play a vital role in providing high-quality care to our clients in their homes.

Responsibilities:
  • Assist clients with daily activities of living, personal care, and respite for caregivers
  • Provide support with meal preparation, recreation activities, and other tasks as needed
  • Develop respectful working relationships with clients and families to meet their care needs
Requirements:
  • Continuing Care Assistant Certification (or currently in school to receive certification)
  • Up-to-date CPR/First Aid Certification and acceptable criminal check and child abuse registry
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
