Job Summary:

Closing the Gap Healthcare Group is seeking a skilled Continuing Care Assistant to join our team. As a Continuing Care Assistant, you will provide high-quality care to clients in their homes, assisting with daily activities, personal care, and other essential tasks.

Key Responsibilities:

  • Assist clients with daily activities, personal care, and respite care
  • Provide recreation activities and meal preparation as needed
  • Develop respectful working relationships with clients and families to meet their care needs
  • Travel to clients' homes within urban and rural settings

Requirements:

  • Continuing Care Assistant Certification or equivalent
  • Up-to-date CPR/First Aid Certification and acceptable criminal check and child abuse registry
  • Valid driver's license and vehicle
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
