Job Summary
We are seeking a skilled Client Support Specialist to join our team at 540 Cedarvale Ave. As a key member of our Supportive Housing Program, you will provide emotional support, feedback, and encouragement to program participants, promoting their participation and well-being.
Responsibilities
- Interact with program participants, providing emotional support and facilitating their involvement in the program.
- Support one-on-one interactions, implementing coordinated care plans, and assisting social workers with case management activities.
- Maintain participant records, monitor progress, and contribute to evaluating Coordinated Care Plans.
- Assist with intake processes, including applicant selection, interviews, and follow-up.
- Collaborate with the team to evaluate program effectiveness, identify areas for improvement, and inform goal setting.
- Support participants in achieving independence while in the program and within the community.
- Liaise with service providers and community organizations to establish networks and secure housing options.
- Recognize, assess, and respond to potential aggressive behavior or crisis situations, informing the team and management when necessary.
- Perform delegated tasks, such as managing medication, arranging transportation, and accompanying participants to appointments.
- Maintain accurate documentation and case notes, ensuring efficient record-keeping.
Requirements
- Diploma in Social Service Work or related education.
- 2+ years of experience working with older adults, mental health, addictions, and street involvement.
- Ability to interact effectively with high-risk, marginalized, and vulnerable individuals.
- Effective de-escalation and boundary-setting skills.
- Excellent communication, interpersonal, and conflict resolution skills.
