Job Summary

Family Services of Greater Vancouver is seeking a skilled Care Coordinator to support our mission of providing exceptional services to vulnerable populations. In this role, you will work collaboratively with clients, families, and community partners to develop and implement personalized care plans that promote health, well-being, and independence.

Key Responsibilities
  • Develop and maintain relationships with clients, families, and community partners to understand their unique needs and goals.
  • Conduct comprehensive assessments to identify client strengths, needs, and service requirements.
  • Collaborate with healthcare professionals, social workers, and other stakeholders to develop and implement care plans that address client needs.
  • Provide education and support to clients and families on healthy living, wellness, and disease management.
  • Facilitate access to community resources, services, and benefits to promote client self-sufficiency.
  • Document client progress, update care plans, and communicate with stakeholders as needed.

Requirements
  • Degree in Social Work, Nursing, or a related field.
  • 2+ years of experience in a care coordination or related role.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Knowledge of community resources, services, and benefits.
