Primary Care Nurse

Vancouver Coastal Health is seeking a skilled Registered Nurse to join our team at the Primary Care Network in Powell River, BC. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities- Assess, plan, implement, and evaluate nursing care for patients/clients and caregiver(s).
- Maintain a partnership with the Primary Care Provider in accordance with the local PCN care delivery model.
- Apply health promotion, chronic disease management, advance care planning, and self-care management principles to conduct assessments, identify problems, provide direct care, and address issues or variances from the plan of care.
- In collaboration with the patient/client and interdisciplinary team, provide continuity of care by liaising with team members, coordinating ongoing care, supporting transitions, and navigating services.
- Act as a nursing resource for the team by applying problem-solving and critical thinking skills and providing information based on nursing theory and practices.
