About the Role

Vancouver Coastal Health is seeking a skilled Community Health Outreach Specialist to join our Priority Populations Outreach Team. As a key member of our team, you will provide support and outreach services to vulnerable and marginalized clients and communities impacted by emerging public health issues.

Key Responsibilities
  • Work collaboratively with an interdisciplinary team to provide harm reduction support, navigate access to pharmaceutical alternatives, and coordinate prescribing access for substance use with community-based clinicians and related support services.
  • Monitor clients' health conditions and environments, arrange and escort clients to appointments, errands, and other activities as required.
  • Create linkages to other community resources and programs, and assist clients in accessing benefits or assistance as appropriate.
  • Administer medication to clients under the direction of a Registered Nurse and provide medication reminders.
  • Liaise and work with community resources and healthcare providers to provide basic health-promoting information and promote development of life-skills to clients.
Qualifications

To be successful in this role, you will require:

  • A diploma in Community Social Services and one year's recent experience in an interdisciplinary healthcare environment working with people with substance use issues and/or those suffering from complex mental health issues.
  • Cardiopulmonary Resuscitation (CPR) certification.
  • A valid BC Driver's license and access to a personal vehicle, as required.
