Position: Community Outreach Specialist

Kílala Lelum Health and Wellness Cooperative is looking for a dedicated Community Outreach Specialist to enhance health and wellness equity in the Downtown Eastside (DTES) through our Relational Care Program. This initiative focuses on providing essential services such as food security, nursing, and culturally-informed care to our members. With approximately 300 individuals benefiting from our food security program and 50 engaged in medication management, our mission is to deliver healthcare through an outreach-based, culturally-sensitive, trauma-informed, and member-focused approach.

Role Overview:

The Community Outreach Specialist will be an integral part of our interdisciplinary health service team, delivering support to members residing in the DTES.

In collaboration with a diverse team that includes primary care, mental health, and substance use professionals, the Community Outreach Specialist will engage with members, coordinate with community care providers, and document observations in partnership with nursing staff. This role is crucial for fostering and maintaining supportive relationships with individuals in the DTES, thereby enhancing their access to cultural connections and healthcare services.

Key Responsibilities:

  • Conduct outreach activities within the community, including the delivery of medications and food supplies.
  • Facilitate access to community resources and services.
  • Coordinate with the integrated care team and local agencies to streamline services.
  • Participate in case planning, assist members with medication management, and accompany them to appointments as necessary.
  • Advocate for the health and psychosocial needs of members.
  • Build and maintain therapeutic relationships grounded in respect and understanding.

Qualifications and Skills:

  • Demonstrated knowledge and experience in providing culturally safe and competent care, particularly for Indigenous populations, with a commitment to ongoing professional development in Indigenous Cultural Safety.
  • Familiarity with trauma-informed practices, harm reduction strategies, and recovery-oriented care.
  • Experience working with individuals facing mental health and substance use challenges.
  • Proficient in conflict resolution, managing aggressive behavior, and applying non-violent crisis intervention techniques.
  • Able to work independently as well as collaboratively within an integrated team.
  • Strong interpersonal skills to establish and maintain effective working relationships with the public and colleagues, while ensuring confidentiality and effective communication.
  • Basic computer skills and familiarity with electronic medical records systems are advantageous.

Working Conditions:

  • Outreach activities will take place in various settings within the DTES, including hotels and private residences, adhering to safety protocols established by the leadership team.
  • Occasional accompaniment to appointments at healthcare facilities may be required.
  • Physical capability to perform job duties, including walking significant distances daily.

Supervision:

This position will be supervised by the Outreach Manager and the Relational Care Lead, with additional support and mentorship from the interdisciplinary healthcare team.
