We are looking for a dedicated Community Outreach Specialist who is committed to advancing health and wellness equity in underserved communities. This role is part of the Relational Care Program at Kílala Lelum Health and Wellness Cooperative, which focuses on providing essential services such as food security, nursing, and culturally appropriate care. Our program serves a diverse membership, ensuring access to vital health resources through a supportive and inclusive approach.

Role Overview:

The Community Outreach Specialist plays a crucial role in delivering support services to individuals in need, working collaboratively within an integrated health service team.

Operating within a multidisciplinary framework that encompasses primary care, mental health, and substance use support, the Community Outreach Specialist will engage with community members, coordinate with various service providers, and document interactions to enhance care delivery. This position is essential for fostering meaningful relationships that improve access to healthcare and cultural resources.

Key Responsibilities:

  • Conduct outreach initiatives to connect with community members; distribute medications and food supplies.
  • Facilitate access to community resources and services.
  • Collaborate with the integrated care team and external agencies to coordinate services.
  • Participate in case planning, assist with medication management, and accompany members to appointments as necessary.
  • Advocate for the health and psychosocial needs of community members.
  • Build and maintain therapeutic relationships grounded in respect and understanding.

Qualifications and Skills:

  • Experience in providing culturally competent care, particularly for Indigenous populations, with a commitment to continuous learning in Cultural Safety.
  • Familiarity with trauma-informed practices, harm reduction strategies, and recovery-oriented care.
  • Experience supporting individuals with mental health and substance use challenges.
  • Ability to effectively manage conflict and utilize non-violent crisis intervention techniques.
  • Capacity to work independently as well as collaboratively within a team environment.
  • Strong interpersonal skills to foster positive relationships with colleagues and community members, while maintaining confidentiality.
  • Basic computer skills and familiarity with electronic medical records systems are advantageous.

Working Conditions:

  • Outreach activities will take place in various community settings, adhering to safety protocols established by the leadership team.
  • Occasional travel to appointments at healthcare facilities may be required.
  • Physical capability to perform job duties, including walking significant distances as needed.

Supervision:

This position will be supervised by the Outreach Manager and will receive ongoing support from the interdisciplinary healthcare team.
