Job Summary

Halton Healthcare is seeking a skilled Social Worker to join our interdisciplinary Step-Up Neuro Rehab team. As a key member of the team, you will work closely with patients, families, and healthcare professionals to provide comprehensive social work services and support the reintegration of patients into their community.

Key Responsibilities
  • Conduct thorough social work assessments to identify patient needs and develop effective care plans.
  • Collaborate with the inter-disciplinary team to address ongoing patient needs and ensure the coordination of community-based support services.
  • Provide education and support to patients and their families to help them navigate the healthcare system and access community resources.
  • Engage patients in goal-setting and problem-solving to promote their socio-emotional health and well-being.
  • Develop and implement strategies to address complex issues and conflicts that may arise during the patient's care journey.
Requirements
  • Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree required.
  • Current registration and membership in good standing with the College of Social Workers and Social Service Workers.
  • Minimum 3 years of experience in a hospital or community health organization, preferably in a neurology or stroke setting.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare professionals.
  • Strong knowledge of community resources and research skills to secure new resources.
